ERS is database-backed for reviewer-room use and reads the selected live evidence feed. The selected evidence pack, session state, classifications, votes, rationale, debrief and audit events are saved as protected records. It does not approve, certify, rate, finance, validate, or replace institutional review.

Your mandate is not to judge whether the programme is desirable, strategic, or well-performing. It is to determine, for each loaded evidence record, what survives reconstruction, what requires validation, and what cannot be reconstructed and must be excluded.
Do not rely on a strong claim or attractive indicator alone. A record that cannot be traced through actor, method, time, place, custody and route cannot be defended at audit.
ERS does not rate, certify, approve, validate or finance. It records reconstruction discipline so a human reviewer can defend the reliance position — or correctly refuse it.
Review failure modes
Four review checks determine whether the evidence record can be defended once reconstruction is tested.
Chain degradation
Attribution thins until an item can no longer be tied to who did what, when and where.
Broken lineage
Lineage to source is missing, so the record cannot be rebuilt from first principles.
Custody fragility
Evidence depended on a partner who has exited with no handover; custody dies with them.
Standard drift
Reviewer standards drifted or routes disagree, so a prior decision can no longer be defended.
